Having to articulate your concerns at the time of your practice visit can sometimes be difficult. It may not be easy to describe the pain or discomfort you are experiencing, especially when it varies, seems to shift location, and when it manifests in different ways and forms.
Our proprietary Concern Sheet helps you structure your observations. Not only does it help you prepare for your treatments, it also helps us design treatments that are most effective for you.

What is the purpose of the Concern Sheet?
Bodywork can only be meaningful if it invites the emotional, physical, and mental changes that are most meaningful to you. Such changes seldom manifest instantly. Instead, they occur gradually, over time, as you progress through your treatment programme.
How, when, and where these changes occur depends on your unique condition. Healing will therefore always be a highly personal affair and will follow a journey that will always be exclusively yours. However, to a trained and experienced therapeutic eye, commonalities can sometimes be identified, which can then be used to further improve and accelerate your treatments.
The information you provide on your Concern Sheet helps us to personalise your treatments to keep them not just effective but also safe and comfortable for you. To us, this information will always reflect your uniqueness. It helps us understand how we can make your treatments most meaningful and relevant to you, and we can work with you best to protect and preserve your self-agency and sovereignty.
How to use the Concern Sheet?
The Concern Sheet is formatted as a PDF, which can only be completed digitally (on your computer).
After downloading a copy of the sheet, reopen it in a standard web browser, such as Safari, Firefox, Chrome, or Edge. Simply navigate your cursor (mouse) to the fields you wish to complete or use the tab function of your keyboard to progress to next questions.
Text fields – To enter a typed response to the description and note fields, simply place your cursor on the field and use your keyboard to enter your response. As most fields are limited to either 650 or 850 characters (as indicated) we ask you to enter your responses concisely. Additional information can be discussed with your therapist at your next visit.
Tick boxes – To enter a ‘yes’ value to a tick box, simply place your cursor on the box and use the left click to add a red-coloured marker in the box. You can remove the marker by repeating this.
Once completed you MUST save your sheet before closing your browser tab or window. The saved version can be send to us by email as an attachment.

When to use the Concern Check Sheet?
We ask you to complete a Concern Check Sheet when you decide to book a first treatment after your Intake Consultation. Once we received your first sheet you may resubmit others periodically, when you believe that this may support your therapeutic progress. We also may ask you to resubmit a copy to keep our treatment records up to date.
How to submit your Concern Check Sheet?
In most cases you can submit your Concern Sheet as an attachment to an email. That may not always work when your hosting provider limits the size of the attachments you can send. If this prohibits you from sending us your completed sheet, simply drag the file from your local computer into the file upload box below.
